Patients deciding between All-on-4 and All-on-6 often worry that the six-implant option means a much harder operation and a longer recovery. The reassuring reality is that the two experiences are far more alike than different. Here is exactly what to expect from each, from the day of surgery to eating normally again.
The surgery: the same workflow, a little more time
Both procedures follow the same path, usually under local anaesthetic with optional IV sedation:
- A CBCT (3D) scan and digital planning of every implant position.
- Removal of any remaining or failing teeth in the arch.
- Placement of the implants — four for All-on-4, six for All-on-6.
- A fixed temporary bridge, usually the same day ("teeth in a day") or within 24–72 hours.
- Healing of 3–6 months, then the final permanent bridge.
The one real difference is time on the day. All-on-4 takes around 1.5–2 hours per arch; All-on-6 takes around 2–3 hours, because there are two more implant sites to prepare and place. That extra time can translate to slightly more post-operative swelling with All-on-6, but it is a difference of degree, not of kind.
Recovery: what the first two weeks feel like
For both procedures, recovery follows a familiar pattern:
- Days 1–3: mild to moderate swelling and discomfort, well controlled with standard painkillers. Swelling peaks around day two or three.
- Days 3–7: swelling settles; most patients feel comfortable and are out and about.
- Weeks 1–2: a soft-food diet to protect the healing implants — soups, eggs, pasta, fish, well-cooked vegetables.
- Weeks 2 onwards: gradual return to a normal diet on the temporary bridge, avoiding very hard foods until the final bridge is fitted.
The honest summary: recovery from All-on-6 is comparable to All-on-4, perhaps a touch more swelling in the first few days. Neither is the ordeal many patients fear.
Bone grafting can change the timeline
One thing that genuinely affects recovery is grafting. Because All-on-6's upright posterior implants need more bone, some patients require a sinus lift or minor graft first — adding cost and 3–6 months of additional healing before the final bridge. The timeline does not differ meaningfully between All-on-4 and All-on-6 — unless grafting is needed, which extends the gap between trips. Some straightforward cases can be completed in a single trip.
