All-on-6 is the reinforced cousin of All-on-4: two extra implants for stronger support across the whole arch. It is the preferred choice for patients with good bone and a heavy bite. Here is how it works.
What is All-on-6?
All-on-6 replaces a full arch of teeth with a fixed bridge supported by six implants instead of four. The two additional fixtures are placed in the posterior region, usually more upright, so there is an implant directly beneath the molar area rather than a cantilevered bridge section. The result is more even load distribution and a built-in safety margin.
Who is All-on-6 for?
All-on-6 is the stronger choice for patients who:
- Have good posterior bone volume and density (so the upright implants can be placed).
- Have a heavy bite or grind their teeth (bruxism).
- Need a longer bridge or have a larger jaw.
- Want the reassurance of redundancy — if one implant is lost, five still support the arch.
The procedure, step by step
- CBCT scan and digital planning, checking posterior bone height carefully.
- Extraction of any remaining teeth.
- Placement of six implants, distributed along the arch.
- A fixed temporary bridge, usually within 24–72 hours.
- 3–6 months of healing, then the final reinforced bridge.
Surgery takes around 2–3 hours per arch — roughly 30–60 minutes longer than All-on-4 — because there are two more implants to place. If your posterior bone is limited, a sinus lift or minor graft may be needed first, which adds cost and healing time.
