What is All-on-6?

All-on-6 replaces a full arch of teeth with a fixed bridge supported by six implants instead of four. The two additional fixtures are placed in the posterior region, usually more upright, so there is an implant directly beneath the molar area rather than a cantilevered bridge section. This can give more even load distribution and, for some patients, an additional margin if one implant were ever lost — though outcomes depend on individual anatomy and treatment planning, not on implant count alone.

Who is All-on-6 for?

All-on-6 is more often discussed for patients who:

• Have good posterior bone volume and density (so the upright implants can be placed).
• Have a heavy bite or grind their teeth (bruxism).
• Need a longer bridge or have a larger jaw.
• Value the additional mechanical margin that six load-bearing points can offer over four, discussed with their clinician against their own anatomy.

The procedure, step by step

1. CBCT scan and digital planning, checking posterior bone height carefully.
2. Extraction of any remaining teeth.
3. Placement of six implants, distributed along the arch.
4. A fixed temporary bridge, usually within 24–72 hours.
5. 3–6 months of healing, then the final reinforced bridge.

Surgery takes around 2–3 hours per arch — roughly 30–60 minutes longer than All-on-4 — because there are two more implants to place. If your posterior bone is limited, a sinus lift or minor graft may be needed first, which adds cost and healing time.

How durable is All-on-6?

Published systematic reviews report high cumulative implant survival for both All-on-4 and All-on-6 over 5 years of follow-up in well-selected cases — the evidence does not show one configuration reliably outlasting the other as a general rule. What All-on-6 can add, for patients whose anatomy calls for it, is more even load distribution under a heavy bite and an extra mechanical margin from two additional fixtures. All-on-6 vs All-on-4 — is the extra cost worth it?

Only if your anatomy calls for it. If you have dense posterior bone and a heavy or grinding bite, the extra £1,500–£2,500 per arch buys a margin that many patients in that position find worthwhile. If your bone is moderate and your bite average, All-on-4 can achieve a comparable fixed result for less — more implants is not automatically the better outcome; the right number is decided on a CBCT scan, not by price.
