All-on-4 vs All-on-6: your questions answered

Not universally. All-on-6 distributes bite force across six implants and can offer an additional mechanical margin, so it is more often discussed for dense bone, heavy bites and longer bridges. All-on-4 needs less bone, generally costs less, and suits a large proportion of patients. The right choice is decided by a CBCT scan, not by price or marketing — more implants is not automatically the better outcome for a given patient.

In Turkey, All-on-6 typically costs about £1,500–£2,500 more per arch than All-on-4 — roughly £6,000–£9,500 versus £4,500–£7,500. In the UK private sector the gap is larger in absolute terms because base prices are considerably higher across the board. These are typical quoted ranges, not audited averages.

Published systematic reviews report high implant survival for both, over 5–10 years of follow-up in well-selected cases — the evidence does not show one configuration reliably outlasting the other as a general rule. All-on-6 generally needs more posterior bone height than All-on-4 because its rear implants are placed more upright. If you have resorbed ridges you may need a sinus lift or graft, or your clinician may recommend All-on-4 instead. A CBCT scan is the only way to know for certain.

Recovery is broadly similar — mild swelling for a few days and soft foods for a couple of weeks. All-on-6 surgery can take somewhat longer per arch because there are two more implants, which may mean slightly more post-operative swelling for some patients.

Both procedures usually involve two visits: a first trip of roughly 5–10 days for surgery and a temporary bridge, then a second trip some months later for the final bridge once healing is confirmed. Some suitable cases can be done in a single, longer trip.

A fixed temporary bridge the same day or within 24–72 hours is common with both All-on-4 and All-on-6, but it is confirmed by the treating clinician on the day of surgery based on implant stability and bone quality — it is not something that can be promised in advance for every case. The permanent bridge is fitted after healing.
