What is All-on-4?

All-on-4 is a protocol for replacing a whole arch of missing or failing teeth with a fixed bridge of 10–14 teeth supported by just four implants. The breakthrough is geometry: the two front implants are placed vertically, while the two back implants are typically tilted at an angle. Tilting can let them grip more bone and, in many cases, avoid the maxillary sinuses (upper jaw) and the inferior alveolar nerve (lower jaw) — which is one reason some All-on-4 patients avoid bone grafting. Whether grafting is needed still depends on individual bone volume and is confirmed on a CBCT scan, not guaranteed by the protocol alone.

Who is All-on-4 for?

All-on-4 suits a large proportion of full-arch patients, especially those who:

• Have moderate bone loss and want to avoid grafting and extra healing time.
• Are tired of loose, uncomfortable dentures and want fixed teeth.
• Want the most cost-effective fixed full-arch solution.
• Have an average bite and good oral hygiene habits.

The procedure, step by step

1. CBCT scan and digital planning of implant positions.
2. Extraction of any remaining teeth in the arch.
3. Placement of four implants — two upright, two tilted.
4. A fixed temporary bridge, usually the same day ("teeth in a day").
5. 3–6 months of healing, then the final zirconia or acrylic-on-titanium bridge.

Surgery typically takes around 1.5–2 hours per arch under local anaesthetic, with optional sedation. Recovery is quick — soft foods for a couple of weeks and mild swelling that settles within days.

How long does All-on-4 last?

With good hygiene and regular reviews, All-on-4 can last well beyond 15–20 years. Published systematic reviews report high cumulative implant survival over 5 years of follow-up in well-selected cases, though survival is a narrower measure than clinical success and figures vary by study — a bare "94–98%" quoted without a source or definition is a marketing shorthand, not evidence.
